If you need help with daily activities and want to stay in the comfort of your own home, our Structured Family Caregiving (SFC) program may be a great option for you.  

This Medicaid program allows a family member or trusted loved one who helps with your day-to-day care to receive financial support for the time and attention they provide. That means you can continue receiving care from someone you know and trust — while getting professional guidance and oversight from our care team at Family Caregivers at Home.

The Structured Family Caregiving program can also be combined with our Medicaid PA Home Health services to provide even more comprehensive, continuous support at home.

 

Medicaid Attendant Care Services

If you need help with daily tasks such as bathing, dressing, preparing meals, or getting around your home safely, Medicaid Attendant Care may be the right fit for you.

Through this program, you can receive personal assistance from a trained caregiver — often a family member — who helps you stay safe, comfortable, and independent at home.

Our team will work with you to coordinate all the details — including eligibility, care authorizations, and required paperwork — so you can focus on living life on your terms.

This service can also be combined with our Medicaid PA Home Health Aide program, giving you additional skilled support and peace of mind when more complex care is needed.

 

Medicaid PA Home Health Aide Services

If your health needs require more hands-on or medically directed care, the Medicaid Prior Authorization (PA) Home Health Aide program can provide the extra support you need — often from someone you already know and trust.

Through this program, a qualified Home Health Aide — which may be a trained family member — helps with daily personal care and other necessary tasks outlined in your doctor’s care plan. It’s designed to keep you safe, comfortable, and well cared for in your own home rather than in a facility.

This service works seamlessly with Structured Family Caregiving or Attendant Care, giving you a complete home care solution with consistent, coordinated support.
